In aesthetic dentistry, the production of dental crowns is practiced mainly by two methods - threshold crowns and thresholdless crowns. In the past, thresholdless crowns have been the choice of many dentists because of the easier fabrication method and the idea that less of the tooth is filed. Unfortunately, technology does not allow the production of thresholdless crowns that fit exactly on the tooth. That is why this type of crowns easily retain food under their edge, which leads to inflammation of the gum and is a prerequisite for the development of caries under the crowns.
Moreover, this inflammation is associated with the development of periodontitis, bone loss and eventually the loss of the tooth itself.
In modern aesthetic dentistry, the only method of making crowns is threshold filing. By means of this filing, a small step is formed around the tooth, and subsequently the fabricated crowns fit perfectly along the contour of the tooth. This allows the patient to clean their teeth easily, which prevents food from getting stuck around the crowns. Threshold crowns are not traumatic to the gum. Their production helps to avoid local inflammation around the tooth, which leads to the development of periodontitis.
Threshold crowns can be of different types depending on the material from which they are made. The bonding (composite or photopolymer veneer, built directly or indirectly with the participation of a dental laboratory, as well as cosmetic corrections of the front teeth with photopolymer/photocomposite/composite) is an aesthetic procedure that achieves the natural restoration of dental defects related to disorders of the shape, color and relationships of the teeth in a few visits (even just one) at an affordable price. This method uses photocomposites as a building material, and this can be done in one or two visits. In the first case, after preparing the tooth surface, the photopolymer is applied in layers and modeled in the dentist's office and the patient leaves with a completed case. In this way, various problems can be restored: a broken tooth or part of it; distances between the teeth; irregular shape - differences in size or shape compared to the surrounding teeth; changed color; extensive caries; dental disorders as a result of bad habits, etc.
The secret of retention in these cases lies precisely in the bonding or adhesive – the system that connects the tooth and filling. In recent decades, adhesive dentistry has developed rapidly in this area of bonding. This is also due to the lasting aesthetic results after the treatment. The adhesive (bonding agent) enters the pre-prepared tooth surface (through etching – acid treatment of enamel and dentin, which roughens them a thousandfold and thus increases their holding contact surface) and binds to it chemically, and subsequently to the photopolymer on top. This is followed by curing – the polymerization of the material with the photopolymer lamp (with ultraviolet light), which takes place in seconds.
The composite material is available in different "colors" and shades, which allows the complete individualization of each tooth compared to the others.
The advantages of the method over the indirect method are the number of visits and the cost of treatment. The disadvantage is the difficult manipulability and not so perfect and lasting aesthetic effect (compared to pure ceramic/pure porcelain).
In the two-session method, the teeth are prepared, then a silicone impression is taken and sent to the laboratory, where an accurate model is cast and the bonding is modeled on it in a similar way to the above, but in a more accessible environment - dryness, manipulability, different points of view etc. In the next visit, the structures are cemented with bonding systems containing the components of the direct method – these are called resin cements. This method is more labor intensive.
Bonding is an aesthetic procedure with considerable durability - about 10-15 years, after which a possible revision is recommended. This is much shorter than ceramic crowns and veneers, but also more affordable.
